There is an excellent Guide for Visiting Away Fans on Derby County’s official club webpage on the Supporters tab.

Page 16 & 17 of this guide provides all the relevant information for disabled fans including Blue Badge parking at the away end and how to book (as spaces are limited this needs to be done asap).
As this guide is so comprehensive, all we need to add is that if you want to book accessible travel to the game at the same cost as travel on the official away travel coaches, please contact the MK Dons Box Office, again asap.
